Why it matters: MASLD is extremely common and has almost no approved treatments, so an effective drug could matter for a very large population but reducing liver fat on a scan isn't yet proof it prevents the cirrhosis and liver failure that make the disease dangerous.
For fatty-liver disease (MASLD), which has almost no approved drugs, the phase 3 SYNCHRONIZE-MASLD trial found a weekly survodutide injection sharply reduced liver fat over 48 weeks. The caveat sits in the design: it was powered for surrogate markers liver fat and body weight not for proof that it resolves liver inflammation (MASH) or reverses scarring, which await biopsy-based trials. Survodutide (Boehringer Ingelheim) is a dual agonist hitting both the GLP-1 receptor (curbing appetite) and the glucagon receptor (helping the liver burn fat). In the trial, 84.2% of treated patients achieved at least a 30% liver-fat reduction vs 24.3% on placebo, 61% normalized their liver fat, and body weight fell up to 12.2% vs 1.0%. Tolerability is a real limit: in the companion obesity trial, gastrointestinal side effects led 19% of survodutide patients to discontinue, vs 2.9% on placebo. Most important limitation: reducing liver fat is not yet proven to prevent cirrhosis or liver failure; this is a commercial drug, and coverage stays independent of that.
